Who was involved or present? (Include names, job titles, or roles if you know them, plus any bystanders).

Was there any follow-up or impact? (Did you report it to anyone else? Did it affect your job duties, schedule, or mental health?)

Are there any supporting materials? (Emails, text messages, shift logs, or photos).

• Protect photo privacy: If you upload screenshots or photos, be aware that original smartphone photos can contain hidden location/time data (EXIF metadata). Taking a screenshot of a photo or document strips out location metadata.
